In the first quarter of 2026, Lexis Nunez owned Ferncrest Manor Living Center, records from the Centers for Medicare and Medicaid Services (CMS) show. The nursing home, located in Orleans County, has operated under Nunez since Jan. 12, 2016.

Ferncrest Manor Living Center is classified as a for-profit nursing facility.

At that time, the center received an overall CMS rating of 1, which falls below the Louisiana nursing home statewide average of 2.5. These CMS ratings are determined by assessments of health inspections, staffing levels, and quality of care metrics for residents.

A 2021 analysis estimates that almost half of people in this age bracket, or close to 28 million, could need long-term services and supports, with more than one-third projected to require nursing home care at some point.

“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance, an insurance education association. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
